They were available within a week. They required me to purchase the metal decking ahead of time and have it available. The decking measurements they gave me were incorrect, so it had to be cut on site. I had to provide a grinder and two cutting disks so that they could cut the metal. After they completed the pour and finish on the concrete, I made the mistake of paying them that day. Three days later I finally removed the forms, since they had not returned to do this. One side of the cap had holes going into the interior of the root cellar under the decking. We were going out of town for a few days, so I waited to call until we returned. During our absence they showed up and picked up their wooden forms. When I called after we returned to mention the problem, they responded that they saw the issue and were waiting to see if I complained. I told him I did want it fixed. One week later I called again and left a message. As of 9/21/2011 there has been no response. I made the repairs myself - $4.00 bag of cement and an hour of work.
