After a few less-than-customer-friendly conversations by phone with other local sandblasters, I was frustrated and feeling belittled about my questions (I don't know anything about sandblasting, people, that's why I called on you 'professionals' for help!). With one last Google search, I found Miller and used their web site's 'contact us' email to ask if they would consider doing this job. The owner replied within an hour to say, 'Yes!' and even provided a rough estimate without even seeing the pieces (and I didn't even ask about price in my email). I dropped the pieces off the next day, was told it would take a week or two (understandably, their industrial work takes precedence), and the price was confirmed. Four business days later, the pieces were ready! Both the owner and the office manager were friendly, accommodating, and seemed genuinely interested in the fireplace pieces and the work. I am very happy with the results.
