Description of Work: The provider did not pay their bills to Forms + Surfaces, Inc. They owe $2729.37 from October 2011 for benches we supplied for Blue Cross Blue Shield. Sharon Burke promised many times to pay the balance due but repeatedly reneged on her promises. Please do not use this vndor. Thank you for your support.
