Did not properly clean out the septic tank. There was scum left on the outlet side, then it appears their hose was clogged by something and they just left such on top of the baffle wall. Our outlet baffle pipe was crushed (do not know if they did this or not), but they should have informed me of this issue. How do I know all this -- I opened up the septic tank cover as I was not there when it was cleaned out. My understanding from speaking with others in the industry is the septic service should include inspection regarding any issue (broken outlet baffle pipe, etc) and back flushing the system to get most of the solid cleaned out, you do not want to clean out 100% as need to leave a little to restart the system - but outlet side should basically be clean. Then any matter that clogs the cleaners hose should surely be removed. Have hired another company to come clean out the septic tank and also fix the obviously broken pipe.
